What's Next?! Workshop Series Survey

Thank you for participating in the What's Next?! Workshop series.  It has been an honor to share this with you.  In an effort to continually improve the program, I ask that you please fill out this survey, answering as honestly as possible.  Your feedback is truly valued, appreciated, and past surveys have added important new dimensions to what is offered.

You may complete this survey anonymously, if you wish to; however, if you are willing to be quoted using your full name in future marketing materials (website, brochures, etc.) please fill in your name at the bottom.  If you wish for only your initials, town, and state of residence will be used as identification, please say "initials only"  after you enter your name at the end.  Thank you in advance for your assistance.

Warmly,

Joanne Lutz

On a scale of 1 - 10 (10 being the best/ most accurate), please rate the first five statements.  The remainder of the survey is designed to be completed in your own "potentially quotable" words.
